Key Responsibilities
  • Conduct thorough assessments of clients to establish individual treatment needs and develop personalized care plans.
  • Maintain open and effective communication with supervisors, colleagues, family members, and other significant individuals regarding client progress.
  • Organize and provide timely updates to supervisors and team members about appointments, home visits, and ongoing evaluations.
  • Coordinate and execute home visits as required, including chart reviews, staff interviews, and data analysis to ensure comprehensive care.
  • Ensure that all treatment plans are implemented effectively, observing client reactions and modifying approaches where necessary.
  • Engage actively in Interdisciplinary Team Meetings to share insights and collaborate on patient care strategies.
  • Prepare and maintain accurate monthly and quarterly progress notes to document client treatment adventures and outcomes.
  • Facilitate training sessions to ensure staff and team members are adept at implementing program protocols successfully.
  • Provide continuous support through follow-up interviews, home visits, and observations tailored to each clientโ€™s evolving needs.
  • Serve as a liaison with the community, helping to promote awareness and accessibility of mental health services.
  • Contribute to the development of training materials and procedures aimed at enhancing individual programs for clients.
Patient Population / Client Focus

In this role, you will have the opportunity to work with a varied client population that includes both adults and youths facing challenges such as anxiety, depression, trauma, relationship issues, and substance misuse. Qualifications
