Key Responsibilities

As a Physical Therapist at Heal Motion Care, you will:

  • Conduct thorough physical therapy evaluations for children and adults, identifying specific needs and developing personalized treatment plans tailored to each patient's goals.
  • Implement and adapt evidence-based modalities and interventions to facilitate rehabilitation, ensuring consistent progress in achieving treatment objectives.
  • Regularly reassess clinical signs and symptoms to evaluate the effectiveness of interventions, adjusting treatment strategies as necessary.
  • Complete all required documentation, including SOAP notes and progress summaries, using our electronic health record system within designated time frames to ensure compliance and continuity of care.
  • Educate patients and their families about the recovery processes, setting realistic expectations and providing guidance on managing challenges during rehabilitation.
  • Meet established performance targets and contribute to business objectives by actively engaging in initiatives that enhance patient care delivery.
  • Collaborate with interdisciplinary teams within Heal Motion Care, participating in case discussions to ensure comprehensive treatment for patients receiving multiple services.
  • Promote and embody the core values of Heal Motion Care, serving as an ambassador for our organizational culture focused on empathy, respect, and excellence.
Patient Population / Client Focus

Our practice serves a diverse patient population, ranging from pediatric to geriatric clients with various physical therapy needs. Qualifications
