Job description

Title:Home Delivered Meals Program Assistant
Department:Nutrition and Senior Centers
FLSA Status:Non-Exempt
Reports To: Home Delivered Meals & Transportation Program Manager
Summary: Assists the manager and coordinator in daily Home Delivered Meal program operations.
Essential Functions:
1. Conducts HDM consumer intake according to waitlist policies and procedures.
2. Conducts in-home assessments for seniors to evaluate their need for HDM and their functions in various areas.
3. Inputs HDM client personal data and program data in Get Care.
4. Conducts in-home and telephone consumer satisfaction surveys.
5. Assists the Manager and Coordinator in collecting program statistics and preparing reports.
6. Assists with the training of workers and drivers.
7. Attends in-service training and meetings.
9. Maintains program records and reports for audit purposes.
10. Works as a backup driver/worker, delivering meals to clients and meal sites as necessary.
11. Performs other duties as assigned.
Qualifications:
1. A college graduate with one year of work experience is required, or a high school graduate with 3 years of experience working with older adults is preferred.
2. Must have and maintain a valid CA driver's license-minimum of two years of driving experience with a satisfactory driving record.
3. Must be interested in working with seniors and adults with disabilities-the ability to work in a team environment.
4. Bilingual in English and Chinese.
5. Good communication, telephone, and organization skills.
6. Must pass a drug test if assigned as a backup driver.
7. Must pass a strength test for physical fitness for the job: 30 lbs from floor to waist; 20 lbs from waist to shoulder, and 10 lbs from floor to overhead.
