Job Description Summary
The Hospice Team Coordinator is a multifaceted role responsible for overseeing key aspects of hospice operations, including patient care coordination, medical records management, and scheduling. This position plays a critical role in ensuring efficient and compassionate care for hospice patients by supporting the needs of the interdisciplinary team, managing patient information, and maintaining accurate records. This role requires meticulous attention to detail, strong organizational skills, and the ability to handle sensitive information with confidentiality and care.
Essential Job Functions/Responsibilities
  1. Assists in the functions of billing, payroll, and patient care management through timely and accurate data entry and system reporting.
  2. Medical Records Management:
    1. Record Maintenance: Oversee the maintenance, organization, and accuracy of patient medical records, ensuring compliance with all relevant regulations, including electronic medical record internal processes.
    2. HIPAA Compliance: Ensure patient confidentiality and data security are maintained in all aspects of medical record management.
    3. Record Retrieval: Facilitate timely retrieval and distribution of patient records as healthcare providers and authorized personnel require.
  3. Scheduling:
    1. Staff Scheduling: Develop and maintain staff schedules to ensure appropriate coverage for patient care.
    2. Coordination: Collaborate with healthcare professionals, nurses, and administrative staff to manage patient appointments and visits.
    3. Addressing Changes: Adjust schedules in response to changes in patient needs and staff availability.
  4. Accurately enters a variety of data into information systems.
  5. Verifies accuracy of data entered through the use of information systems reports.
  6. Prepares for IDT Meeting, and assists in facilitating the meeting, including ensuring completion of documentation requirements.
  7. Assists in the preparation of monthly billing by ensuring accuracy and completion of the medical record for compliance.
  8. Alerts appropriate management team members regarding late or missing documents required for data entry.
  9. Answers telephone lines promptly and efficiently. Responds to message content appropriately, as instructed by the supervisor. Records telephone calls onto Call Log.

10. Assist in preparing reports and compiling data for audits and quality improvement initiatives.
11. Maintains confidentiality of patient and organization information at all times.

Position Qualifications
  1. At least two (2) years' experience in health care data entry, preferably in hospice care information systems.
  2. Knowledge of healthcare regulations, including HIPAA, and experience in medical records management.
  3. Strong organizational and communication skills.
  4. Proficiency in scheduling software and electronic health record (EHR) systems.
  5. Ability to manage multiple responsibilities and adapt to changing priorities.
  6. Strong interpersonal skills, and ability to work independently, and as part of a team.
  7. Compassionate and patient-centered approach to work.
  8. A high school graduate or equivalent, two (2) years college preferred.
