This full-time position operates within a remote setting, allowing our counselors the flexibility to reach clients in an outpatient office environment without the constraints of a traditional clinical space. As a Licensed Professional Counselor at Alameda Health Care, your core responsibilities will include:

• Conducting comprehensive mental health assessments to identify and address the unique needs of each patient.
• Facilitating open communication between patients, family members, guardians, and interdisciplinary team members to ensure a cohesive understanding of individual progress and treatment plans.
• Scheduling and conducting appointment follow-ups, home visits, and observations to enhance patient engagement and monitor ongoing treatment effectiveness.
• Participating in Interdisciplinary Team Meetings, contributing insights and recommendations for individualized treatment strategies.
• Maintaining thorough and organized records, including monthly and quarterly progress notes, to support the continuity of care and facilitate easy access for other team members.
• Acting as a liaison with the public and mental health community, promoting development goals and collaboration between stakeholders.
• Coordinating training and supervision for fellow staff members to ensure comprehensive implementation of treatment plans and documentation practices.
• Providing ongoing support through follow-up interviews and home visits to assess and adapt care as needed.

Your role will encompass serving a diverse client base, including both adults and youth, each with varying mental health needs. You will specialize in delivering tailored counseling services, with a focus on conditions such as anxiety, depression, trauma, substance misuse, and family dynamics. With your expertise, you will empower clients to navigate their challenges and pave their path toward recovery and wellness.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• Must hold an active and unrestricted licensure as a Professional Counselor (LPC) in the state of Georgia.
• Minimum of two years of relevant counseling experience in a clinical setting, demonstrating your capability to handle various mental health issues.
• A strong foundation in legal and ethical standards related to counseling practices.
• Exceptional communication skills with an ability to cultivate rapport with diverse clientele.
• Proficiency in collaborating effectively with a multidisciplinary healthcare team.
• Familiarity with electronic medical record (EMR) systems and telehealth technologies.

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance abuse counseling is highly regarded.
• Prior experience in developing training materials for staff and contributing to program improvements will be an asset.
