Key Responsibilities:

  • Complete comprehensive in-home patient assessments

  • Develop care plans under primary care providers' orders

  • Provide direct patient care per the plan of care and scope of practice

  • Ensure accurate and timely documentation

  • Participate in QAPI program for top-notch patient care

  • Coordinate interdisciplinary home health services

Qualifications:

  • Registered Nurse (RN) licensure

  • Home Health and/or Hospice experience preferred

  • Current BLS certification from AHA

  • Licensed driver with an insured automobile in good working order
