Role

HCS License Vocational Nurse


Reports to:

Medical Case Manager  (First Point of Contact)

RN (Second Point of Contact)

Program Manager (Third Point of Contact)


Duties and Responsibilities

The LVN works with the RN, Foster Providers, and Direct Care Staff to ensure the health and safety of each client. The focus of this position is monitoring the consumer’s use of medications, monitoring health data and information, and assisting in the health care aspects of the consumer’s daily needs.


Primary responsibilities include:

  • Adhere to the guidelines set forth by the Board of Nursing (BON) and HCS Principles;
  • Complete Face-to-Face Focus Assessments
  • Administer medications;
  • Ensure Proper Storage of Medications at the group homes 1- lock for routine medications, 2- locks for narcotics. Be sure to sign the Storage of Medication Form
  • Monitor the consumer’s use of medications;
  • Monitor consumers’ health data and information;
  • Provide wound care when needed to clients
  • Complete Bi-Weekly Skin Checks and as needed
  • Make client medical, dental, psychiatry, specialty, transportation  appointments
  • Transport and/or accompany clients to appointments as needed
  • Assist the consumer in securing emergency medical services;
  • Perform health care procedures ordered/prescribed by a physician/medical practitioner as required by standards of professional practice or law to be performed by licensed nursing personnel;
  • Monitor tasks delegated in accordance with state law to other service providers;
  • Review Medication Logs for gaps and errors- educate staff inside of WhatsApp *
  • Assist with monitoring PRN usage and documentation of effectiveness
  • Support RN during nursing assessments by gathering client history or helping with forms
  • Reinforce unlicensed personnel/ caregiver training after RN delegation 
  • Complete tasks that cannot be delegated as determined by the RN
  • Observe and report changes in condition to RN 
  • Provide RN support as needed and when requested
  • Review Bowel Movement Tracking Sheet every Monday and Thursday
  • Communicate any missing medications for the month to the pharmacy
  • Complete Monthly Medications check in at each group home (label the blister pack left top corner with a sharpie with medication time Example: (7a, 11a, 3p, 7p, 11p.) Rubber Band the blister packs together by timeframe Example: (7a, 11a, 3p, 7p, 11p.) Take any medications that are not on the clients MAR form the group home. 
  • Complete and Review Service Delivery Logs with RN for all nursing by Friday of each week
  • Send all Service Delivery Logs from the prior week (Sunday to Saturday) in by Monday at 12:00 pm. Important: To avoid a delay in service. 
  • Attend weekly ITD meetings as needed 
  • Complete Medication Training for all Direct Care Staff, as needed; 
  • Give first dose of new medication when medication delegation has been determined by the RN;
  • Agree to be on call after office hours as needed to assist the clients;
  • Complete Quarterly Medication Reviews;
  • Order medication as needed for the Group Homes;
  • Make sure a 7 day supply of all medications are on hand at the Group Homes;
  • Respond appropriately to all consumer crises;
  • Participate in scheduled department and all staff meetings;
  • Keep license current and in good standing.
  • Other duties as assigned.


Qualifications

Certifications, Licenses, or Registrations:

  • Current certification from the Texas Board of Nurses.
  • Current certification in CPR and First Aid;
  • Dependable transportation, valid driver's license and automobile liability insurance;
  • Recent TB test.


Skills and Experience:

  • At least one year of experience in home health;
  • At least one year of experience working with individuals with disabilities;
  • Must have working knowledge of accepted nursing procedures for individuals with disabilities;
  • Familiarity with HCS regulations and contract standards of the program;
  • Must have team-building skills in order to effectively work with others and provide quality services;
  • Must exhibit good relationship-building skills with consumers and their families;
  • Computer literacy in word processing


Pay and Schedule Details:

  • LVN Service Delivery Rate: $29.69/ per billable hour
  • LVN Relief: $100/day
  • Mileage: 0.75/per mile (Complete Mileage Form)
  • Paid Weekly
