Job description

Position Summary: Responsible for providing medically necessary behavioral health services to patients in relation to behavioral, interpersonal, alcohol, or substance use concerns. Identifies and assesses individual and group counseling needs, develops and implements treatment plans for counseling. Communicates frequently with the clinical team, which may include ancillary service providers and the patient's identified treatment supports to provide effective care services. Ensures that all activities and documentations are performed timely, accurately, and in compliance with applicable behavioral health regulations and standards.
Critical Tasks:
  • Conduct comprehensive and annual assessments with patients to determine need for services.
  • In collaboration with patients, guardian, advocate, ancillary service provider and/or patient identified supports, complete individualized service plans based on the comprehensive assessment.
  • Provide services in the office, home, community and other facilities as needed
  • Provide individual, family and group counseling services to patients.
  • Respond to crisis situations, providing immediate support and guidance to patients in distress.
  • Manage their caseload and use available resources to ensure positive treatment outcomes.
  • Maintain accurate and timely medical records, including progress notes, assessments and service plans in accordance with GRHC, state and federal requirements.
  • Educate patients about mental health and wellness, and advocate for their needs within the healthcare system.
  • Coordinate care regularly with individual patient's identified clinical team and communicates as appropriate with patient referral sources.
  • Stay current with evidenced based and best practices in counseling and complete all required training.
  • Adhere to GRHC adopted ethical and professional standards, maintaining confidentiality and building trust with patients.
  • Provide case management duties as needed.
  • Meet regularly with their immediate supervisor for supervision.
  • Follow all state, federal and tribal laws as well as GRHC policies and procedures.
  • Demonstrate the ability to clearly communicate, both orally and in writing, while performing all essential functions.
  • Work with all ages and populations of patients.
  • Will have to work with general mental health issues but will specifically work with substance use, grief and loss, trauma and crisis situations.
  • Work with all GRHC staff and GRIC departments in a respectful and collaborative manner.
  • Perform other special projects and duties as assigned.

Required Qualifications:
  • Masters' Degree in Social Work, Counseling, Substance Use/Addictions or closely related field of study from an accredited university or college.
  • Current independent license from Arizona Board of Behavioral Health Examiners as either LPC, LCSW, LMFT, LIAC required; or moving towards independent licensure with respective associate license.
  • Prefer two years counseling and/or therapist mental health and/or substance use/addictions treatment experience.
  • Arizona Department of Public Safety Level Fingerprint Clearance Card required.
  • Current and continuing CPR/FA and/or BLS training and certifications preferred.
  • Demonstrated ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
