Description
This schedule includes weekday shifts, two evening shifts, and a four-hour shift on Saturdays each week. Hours Include - Tuesdays and Thursdays. 10am-7pm and Saturday. 8am-12pm.
Position Summary:
Case Managers provide case management services to targeted demographics per contract requirements. Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and family's comprehensive needs through communication and resources to promote well-being.
Qualifications:
  1. Case Manager I- High school diploma or GED required.
  2. Case Manager I -Bachelor's degree in Counseling, Social Work or related field preferred. Equivalent experience accepted.
  3. Case Manager II - Bachelor's degree in Counseling, Social Work or related field required.
  4. Knowledge of community resources preferred.
  5. Bilingual preferred.
  6. A valid California driver's license is required of staff who have regular driving duties and the driving record must meet the requirements of their position and be approved by the agency's insurance company.

Essential Job Functions:
  1. Operate an agency or personal vehicle for work-related activities which may include, client visits, meetings, trainings, or other agency-related appointments.
  2. Ensure compliance with driving requirements, including maintaining a valid driver's license, current vehicle registration, and proof of insurance as required by agency policy.
  3. Ability to communicate ideas in oral and written form.
  4. Ability to instruct in group and individual settings.
  5. Physical, mental, and emotional health adequate to perform duties.
  6. Must possess knowledge of mental health programs and community resources.
  7. Adhere to all applicable legal and ethical standards
  8. Protect and honor individual integrity through confidentiality.
  9. Ability to work in a harmonious manner with subordinates, supervisors, and others.
  10. Ability to work effectively with cultural/ethnic diversity.

Responsibilities:
  1. Provide access, advocacy, resources and linkage to clients.
  2. Attend community events, provide outreach services to include identifying areas that need to be strengthened, in addition to setting and reaching targeted goals. May need to work evenings and weekends as needed.
  3. Administer program measurement tools and surveys to track and report program data and outcomes.
  4. Interact productively within a multi-disciplinary team and other community organizations.
  5. Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
  6. Responsible for preventing and controlling infection.
  7. Responsible for maintaining a culture of quality and safety.
  8. Effectively use de-escalation skills and techniques for crisis prevention
  9. Properly administer the C-SSRS to help prevent suicide.
  10. Interaction is appropriate and sensitive to client's culture. Interaction is appropriate with client's developmental level.

Other:
  1. Attend all required meetings and training.
  2. Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
  3. Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.Other duties as assigned.
