Registered Nurse (RN) - Per Diem NOC Shift
Garden City Healthcare Center
Location: Modesto, CA
Pay Range: $40.00 - $45.00 per hour (DOE)
Shift: Per Diem / Overnight
Shift Differential: Additional NOC differential provided
Status: Full-Time
Setting: Skilled Nursing Facility (SNF)
General Purpose
The primary responsibility of the Registered Nurse (RN) is to supervise and provide direct nursing care during the assigned NOC shift in accordance with current federal, state, and local regulations. The RN ensures the highest degree of quality care is maintained and supports safe, effective overnight operations of the nursing department.
Essential Duties and Responsibilities
Administrative & Leadership Functions
  • Assist the Director of Nursing (DON) and/or ADON with oversight of nursing services during the NOC shift
  • Ensure nursing staff comply with facility policies, procedures, and job descriptions
  • Participate in quality assurance activities, audits, and facility surveys as requested
  • Complete required documentation, reports, and incident/accident reports
  • Protect resident confidentiality and report any unauthorized access to information systems
Personnel & Supervision
  • Ensure adequate RN/LVN and CNA staffing levels throughout the shift
  • Supervise, support, and delegate duties to nursing staff
  • Conduct rounds to ensure quality care and compliance with nursing standards
  • Promote a calm, respectful, and resident-centered environment
Nursing Care Functions
  • Provide direct nursing care, including medication administration and treatments
  • Monitor residents for changes in condition and notify physicians and families as needed
  • Admit, transfer, and discharge residents as required
  • Review nursing documentation for accuracy and completeness
  • Ensure infection control, safety, and emergency procedures are followed
  • Assist with care planning, MDS coordination, and discharge planning
Safety, Infection Control & Equipment
  • Ensure compliance with infection control and safety standards
  • Monitor use and maintenance of nursing equipment and supplies
  • Report hazardous conditions, equipment issues, and safety concerns promptly
Supervisory Responsibilities
  • Assist with the overall supervision and management of nursing staff during the assigned shift
  • Delegated authority, responsibility, and accountability for assigned duties
Qualifications
Education & Experience
  • Graduate of an accredited nursing program
  • Minimum 2 years of nursing experience, preferably in a skilled nursing or long-term care setting
  • Training in rehabilitative and restorative nursing practices preferred
Licensure & Certifications
  • Current, active, unencumbered California RN license
  • Current CPR certification
  • Knowledge and experience with PointClickCare (PCC) preferred
  • Must maintain all required continuing education and licensure in good standing
Skills & Abilities
  • Strong leadership, clinical judgment, and decision-making skills
  • Ability to work independently during overnight hours
  • Excellent communication and organizational skills
  • Compassionate, patient-focused, and team-oriented
Physical Demands
  • Ability to stand, walk, lift, push, and pull up to 25 pounds
  • Ability to respond to emergencies, including resident evacuation if necessary
  • Ability to cope with physical, mental, and emotional demands of the role
