As a Home Health Physical Therapist, you'll provide individualized care in the comfort of your patients' homes, helping them regain strength, improve mobility, reduce pain, and restore confidence.

You'll have the freedom to create personalized treatment plans while building meaningful relationships and seeing the direct impact of your care.

What You'll Do
  • Complete evaluations, reassessments, and discharge visits
  • Develop individualized plans of care based on patient goals
  • Provide skilled physical therapy services in patients' homes
  • Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety
  • Monitor patient progress and modify treatment plans as appropriate
  • Collaborate with physicians and the interdisciplinary care team
  • Complete timely and accurate clinical documentation
Qualifications
  • Active Virginia Physical Therapist License Required
  • Valid Driver's License and Reliable Transportation
  • Strong Clinical Assessment and Documentation Skills
  • Excellent Communication and Organizational Skills
  • Home Health Experience Preferred but Not Required
  • OASIS Experience is a Plus
  • Passion for Delivering Exceptional Patient-Centered Care
