Job description

We are a growing, physician-owned medical group expanding in Green Bay, WI seeking an experienced Nurse Practitioner to join our team in a Skilled Nursing Facility (SNF) setting. This role focuses on post-acute and rehabilitative care, including orthopedic recovery, stroke management, and pain management, working alongside a collaborative lead provider. 

Key Responsibilities: 

  • Provide comprehensive medical care to SNF patients, including assessments, diagnosis, and treatment planning 
  • Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions 
  • Ensure accurate and timely documentation in EMR 

Required Qualifications: 

  • Active Nurse Practitioner (NP) license in Wisconsin
  • Active DEA license (or ability to obtain prior to start) - Required 
  • Minimum 1 year of clinical experience as an NP (SNF, geriatrics, internal medicine, or rehab experience strongly preferred) 
  • Experience managing adult/geriatric patient populations 
  • Strong clinical decision-making and time management skills 
  • Comfortable working independently in an SNF setting 

Preferred Experience: 

  • Skilled Nursing Facility (SNF) experience 
  • Background in internal medicine, geriatrics, or physical medicine & rehabilitation (PM&R) 
  • Experience with orthopedic, stroke, or pain management patients 

Schedule & Setting: 

  • Monday through Friday 
  • Full-Time or Part-Time  
  • No nights, no call, no weekends

Physical Requirements: 

  • Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer. 
  • Must be able to lift up to 50 pounds. 
  • May be exposed to bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards. 
  • Must wear the required PPE per company and/or facility requirements. 
  • Must comply with facility vaccination requirements.
