Job description

Job Details
ChristianaCare is currently seeking a part-time hybrid advanced practice provider (PA/NP) for the ChristianaCare ED callback team. In this role you will work collaboratively within the service line and with other departments to ensure timely follow-up of ED results. The APP will utilize established protocols to determine the need for further evaluation, testing, or treatment and will communicate this information directly to patient and/or other healthcare providers.
This is a part-time dayshift position covering 40 hours biweekly with flexibility to allow for some of these hours to be done virtually. The selected candidate will work alongside other callback providers to provide coverage 7 days a week including holidays. This role oversees results for CCHS' Delaware and Maryland campuses.
Principal Duties and Responsibilities:
  • Review all results (lab and radiology) for patients who are discharged from a CCHS Emergency Department.
  • Contact patients to inform of pertinent post-discharge results per callback guidelines and perform necessary follow-up actions which include, but are not limited to, prescribing medications, coordinating outpatient follow-up care, and patient education
  • Receive and manage incoming calls from patients and pharmacists to assist with visit related questions and prescription questions/medication adjustments. Return voicemail messages received during off hours.
  • Coordinate transparency for blood and body fluid exposures involving non-CCHS healthcare professionals seen in the Emergency Department
  • Initiate referrals to appropriate resources and services, promoting a collaborative approach to patient care
  • Communicate with other departments/providers as needed (ie: radiology, lab, inpatient teams, specialists, or family physicians)
  • Document in EHR to maintain complete and accurate record of follow-up care
  • Work with the ED Administration residents and ED PA Fellows to educate them on the workings of the callback role
  • Perform additional callback related duties as they arise
Requirements:
  • Graduate of an accredited Physician Assistant Program or Nurse Practitioner Program
  • National certification as a Physician Assistant or Nurse Practitioner
  • Must possess an unrestricted license in the state of Delaware and be license eligible in the state of Maryland
  • Must possess prescriptive authority and DEA licensure in the state of Delaware and be license eligible for prescriptive authority and DEA licensure in the state of Maryland
  • BLS certification

Annual Compensation Range $125,361.00 - $161,327.00
This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
