Job description

Position Summary
The Lead Housing Case Manager will work with clients in the emergency shelter and assist clients with transitioning into permanent housing. The Lead Housing Case Manager will assist clients with obtaining documents needed for housing, locating appropriate housing, increasing income, and remaining successfully housed. The Lead Housing Case Manager is a senior role that will be tasked with the most challenging cases and providing mentorship to other Case Managers.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
  • Coordination of services for client
  • Ability to provide independent life skills training, group education, and family activity coordination.
  • Assist clients with gathering essential documents
  • Locate housing opportunities in the community
  • Maintain appropriate client files and complete documentation within the required timeframe
  • Provide coaching and guidance to Case Management team
  • Support Case Managers in meeting housing outcomes
  • Maintain accuracy of client rosters and records
  • Support the program staff and functioning in the absence of the Program Manager

Minimum Qualifications (Knowledge, Skills, and Abilities)
  • Bachelors Degree in social services or closely related field, preferred
  • 3 years experience in housing related social services
  • Clean driving record
