Primary Care/Functional Medicine Providers

Eugene, Oregon, United States

Our mission is to deliver a personalized, integrative care model that combines traditional and functional medicine to address the root causes of illness and empower patients to live healthier, more vibrant lives. Our multidisciplinary team supports patients through preventative primary care, advanced diagnostics, hormone optimization, nutrition, lifestyle coaching, and evidence-based therapies.

We are seeking a passionate and skilled Medical Provider to join our growing team. This role is ideal for a provider who thrives in a supportive, forward-thinking environment and is committed to whole-person care. You will provide comprehensive medical services, foster lasting relationships with your patients, and collaborate with a high-functioning care team to drive optimal health outcomes.

Responsibilities and Duties:

  • Provide direct patient care through office visits, telehealth, and occasional same-day appointments
  • Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
  • Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
  • Educate and empower patients on preventive health and functional medicine principles
  • Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
  • Document visits in the EHR (NextGen) in a timely and compliant manner
  • Participate in team meetings, quality improvement initiatives, and training sessions
  • Support clinic operations by contributing to a positive, team-oriented environment

Required Qualifications

  • Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO (or eligibility to obtain)
  • Board certification in Family Medicine, Internal Medicine, or applicable specialty
  • Minimum 2 years of clinical experience preferred (functional/integrative medicine experience is a plus)
  • Strong interest in preventive, functional, and lifestyle medicine
  • Excellent communication and patient relationship skills
  • Comfortable working in a fast-paced, collaborative environment
  • Proficient in using electronic health records (NextGen experience is a plus)
