Job Summary
Our client is seeking an Internal Medicine Registered Nurse to provide comprehensive nursing care to adult patients. The primary responsibilities include assessing patients' health needs, developing individualized care plans, and coordinating care with other healthcare providers.
Key Responsibilities
  • Provide comprehensive nursing care to adult patients within an Internal Medicine outpatient practice.
  • Assess patients' physical, emotional, and social health needs while developing individualized plans of care.
  • Triage patient concerns both in person and over the phone, ensuring appropriate follow-up and provider communication.
  • Administer immunizations, medications, respiratory treatments, and other nursing interventions.
  • Educate patients on chronic disease management, medications, preventive care, and healthy lifestyle modifications.
  • Coordinate care with primary care providers, specialists, pharmacies, and community resources.
  • Manage clinic workflow by reviewing schedules, preparing patient charts, and coordinating same-day appointments.
  • Review laboratory and diagnostic results, communicate abnormal findings, and facilitate timely follow-up.
  • Assist with medication refills, referrals, prior authorizations, and care coordination.
  • Document patient encounters, assessments, education, and interventions accurately within the EMR.
  • Participate in quality improvement initiatives, interdisciplinary meetings, and community health programs.
  • Serve as a clinical resource for medical assistants and ancillary staff while promoting a collaborative team environment.

Qualifications
  • Active Registered Nurse (RN) license in Rhode Island.
  • Minimum 1 year of RN experience in Internal Medicine, Adult Primary Care, Family Medicine (adult-focused), Community Health, or Ambulatory Care.
  • Recent experience caring for adult patients with acute and chronic medical conditions is required.
  • Strong knowledge of chronic disease management including diabetes, hypertension, COPD, asthma, heart disease, and other common adult health conditions.
  • Experience with medication administration, immunizations, patient triage, and patient education.
  • Proficiency documenting within an Electronic Medical Record (EMR).
  • Ability to work efficiently in a fast-paced outpatient clinic.

Skills
  • Technical skills: Proficiency with EMR systems, preferably Epic.
  • Soft skills: Strong communication, ability to work collaboratively, and efficiency in a fast-paced environment.
  • Bilingual, especially Spanish, preferred.
  • Experience working with underserved or diverse patient populations preferred.

Additional Requirements
  • Must be available to work Monday, Wednesday, and Friday from 8am-5pm, and Tuesday and Thursday from 11am-8pm.
