Job Summary

The APRN/PA - Virtual Health functions at a medical level of care to serve patients with acute, chronic, or preventive health needs via Virtual Primary Care (VPC) and Neighborhood Care Site (NCS) programs. Clinical responsibilities include assessment, diagnosis, monitoring, and treatment delivered exclusively through virtual health modalities, including visits for patients who are physically present at organizational clinical sites and supported by onsite staff.

The APRN/PA participates as part of an interdisciplinary team to provide safe, highquality, costeffective, and evidencebased care. The APRN/PA demonstrates the highest levels of professionalism and upholds the mission, vision, and values of Advocate Health.

This job is remotebased but requires access to nearby organizational sites and/or an appropriate home office setup. The APRN/PA will provide care within their scope of practice as outlined by applicable state law, licensure, regulations, institutional policy, privileges, and practice agreements.

Job Description

Clinical Responsibilities

  • Assesses, diagnoses, and determines or alters treatment and management plans appropriate for age, acuity, and clinical condition. This includes ordering, performing (where applicable), and interpreting appropriate diagnostic studies and prescribing pharmacologic and nonpharmacologic interventions and therapies.
  • Provides care to patients through synchronous virtual health visits, including:
    • Virtual Primary Care visits (provider and patient both remote)
    • Neighborhood Care Site visits, where the provider is virtual and the patient is physically present at a clinic supported by onsite staff
  • Manages acute, chronic, and preventive health conditions based on clinical indication, evidencebased practice, cost effectiveness, and assessment of risks, benefits, and alternatives.
  • Provides health promotion, disease prevention, disease management counseling, and education for patients and families.
  • Manages patients as part of an interdisciplinary team and within scope of practice. Seeks physician or other healthcare team member consultation or referrals as appropriate. Escalates need for more emergent or specialized care when clinically indicated.
  • Maintains accurate, complete, concise, and timely documentation in the electronic medical record. Substantiates and submits professional services consistent with compliant coding and billing practices.
  • Facilitates consistent, coordinated care and clear communication among all members of the healthcare team, including virtual care team members and onsite clinical staff supporting Neighborhood Care Site visits.
  • Performs procedures in accordance with specialty practice, competency, and granted privileges (where applicable).

Other Responsibilities

  • Participates in quality, safety, peer review, and performance improvement initiatives, organizational and/or departmental meetings, committees, workgroups, and peer review as required.
  • Participates in education and onboarding of new team members, students, and other healthcare professionals.
  • Seeks experiences to maintain and develop clinical and professional skills and to advance the profession within and outside the organization.
  • Maintains standards of productivity, access, facetoface (virtual) time, and quality metrics to ensure optimal, safe, and timely patient care delivery.
  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served, including understanding principles of growth and development across the lifespan and the ability to assess, interpret, and act on agespecific patient needs as outlined in departmental policies and procedures.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED

License / Registration / Certification Required

  • Active RN license and APRN/APRNFPA/APNP or PA license in Illinois and Wisconsin, or ability to obtain prior to start
  • Active national board certification appropriate to clinical practice and populations served
  • Active DEA registration prior to hire
  • If Illinois practice: active Illinois Controlled Substance License prior to hire
  • Active BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire

Education Required

  • Master's Degree in Nursing (or equivalent graduate degree for Physician Assistants)

  • Demonstrated high level of clinical proficiency and excellent clinical decisionmaking skills

  • Must have functional speech, vision, touch, and hearing
  • Must be able to:
    • Lift up to 50 lbs from floor to waist
    • Lift up to 20 lbs over the head
    • Carry up to 40 lbs a reasonable distance
    • Operate all equipment necessary to perform the job

  • Active PA license(s) in state(s) of practice, AND
  • Active PA national board certification from National Commission on Certification of Physician Assistants.
  • All APRN and Physician Assistant applicants must have:
