This full-time position operates onsite at our home care facility, providing you the opportunity to interact closely with patients in a supportive, state-of-the-art clinic. We work from Monday to Friday, 8 AM to 3 PM, ensuring a balanced work-life dynamic that promotes both professional fulfillment and personal wellness. Our facility is equipped with the latest technology and resources, ensuring that you have access to the tools necessary to deliver top-notch care.

• Conduct thorough Physical Therapy evaluations for pediatric and adult patients, identifying specific needs and conditions.
• Develop and implement personalized, evidence-based treatment plans tailored to the individual goals of each client.
• Regularly assess clinical signs and symptoms, adjusting treatment plans as necessary to ensure clients are on the path to recovery.
• Maintain meticulous documentation, including SOAP notes, progress summaries, and care plans, utilizing our electronic health record system efficiently.
• Provide education and support to patients and their families about the rehabilitation process and potential challenges to enhance understanding and commitment.
• Achieve minimum utilization goals and contribute towards business objectives while aligning efforts with the broader goals of Ensemble Therapy.
• Engage in collaborative care by working alongside peers from various disciplines to coordinate services for patients with multifaceted needs.

Our clients range from children requiring rehabilitative services post-injury or surgery to adults and seniors seeking support for chronic orthopedic conditions. • Must possess a Doctor of Physical Therapy (DPT) degree from a recognized, accredited institution.
• Current and unrestricted physical therapy licensure in the state of New Jersey.
• Ability to deliver evaluations and treatments effectively in-person and adapt to remote telehealth services when clinically appropriate.
• A strong passion for working with a wide range of patient populations, understanding their varying backgrounds and recovery needs.
• Flexibility and agility in a dynamic healthcare environment to adapt to changing patient needs and clinic demands.
• Collaborative mindset focused on teamwork and providing exceptional customer service.

• Prior experience in an orthopedic physical therapy setting is preferred.
• Competency in utilizing electronic health record systems for documentation.
• Experience working with multidisciplinary teams to optimize patient outcomes.

The salary range for this position is competitive at USD 80,000 to 100,000 per year, complemented by a comprehensive benefits package.
