Key Responsibilities

• Conduct thorough assessments of clients' mental health needs and develop individualized treatment plans tailored to their specific conditions.
• Collaborate with other healthcare providers within the hospice setting to ensure integrative care and address the emotional and psychological needs of patients.
• Facilitate ongoing clinical discussions and case reviews with team members, fostering an environment of shared expertise and continual improvement.
• Provide education and resources to clients and their families about mental health issues, caregiving, and available support systems.
• Implement evidence-based therapeutic interventions, including solution-focused therapy and other modalities that enhance treatment effectiveness.
• Document all clinical interactions and treatment progress in accordance with organizational policies and regulatory requirements.

Patient Population / Client Focus

This role primarily focuses on clients experiencing a wide range of mental health challenges related to terminal diagnoses and end-of-life issues. You will engage with a diverse population, providing compassionate support to both clients and families during difficult transitions. Understanding the unique emotional and psychological needs of patients in hospice care is paramount to your success in this role.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• A current and valid LCSW license in the state of California is mandatory.
• At least two years of clinical experience in mental health services, preferably in hospice or similar settings.
• Proficiency in solution-focused interventions and strong interpersonal communication skills.
• A commitment to continuous professional development and team collaboration.

Preferred Experience

• Experience working within a hospice or end-of-life care context will be highly regarded.
• Familiarity with electronic health records (EHR) systems for documentation and administrative tasks.

Compensation and Benefits
