Social Services Assistant needed for our beautiful Luxe at Wellington Health and Rehabilitation Center!!

The Social Services Assistant assists the Director of Social Services in the planning, developing, organizing, implementing, and evaluating our customers.

QUALIFICATIONS for the position of Social ServicesAssistant:

  • Bachelor’s in Social Work is preferred. A Bachelor’s Degree in human services may also be considered.
  • Previous experience in the field required. Previous long term care experience strongly preferred.
  • Needs to possess any Certifications/Licensures required by State regulations.

RESPONSIBILITIES of the Social Services Worker include, but are not limited to:

  • The Social Services Worker assists with planning and implementing a comprehensive Social Services program which provides for counseling and other support services for customers and legal representatives in the Center.
  • Identifies medical-related social needs of customers, provides appropriate services to meet the individual, as well as collective needs of customers, and maintains records relating to the customers’ social work needs and care.
  • Consults with the Director of Social Services and other department heads regarding interdisciplinary issues, as well as maintenance of appropriate records.
  • Works directly with customers and legal representatives experiencing personal and environmental difficulties or concerns related to the customer’s physical or emotional condition.
