This full-time position includes a one weekend per month commitment.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including medication administration, wound management, and vital signs monitoring.
  • Conduct thorough patient assessments and develop individualized care plans aligned with physician orders and patient needs.
  • Educate patients and their families on disease management, medication usage, and health maintenance strategies.
  • Coordinate with physicians, therapists, social workers, and other healthcare professionals to ensure seamless care delivery.
  • Document all patient interactions accurately using electronic health record (EHR) systems such as Epic or Cerner while adhering to HIPAA regulations.
  • Perform specialized procedures such as IV insertion, catheter care, ostomy management, and sterile dressing changes following aseptic techniques.
  • Monitor for signs of acute pain or deterioration and respond promptly with appropriate interventions or triage protocols.

Requirements

  • Valid RN license in the state of practice with current CPR certification (Basic Life Support).
  • Proven experience in home health care or related settings such as hospitals, nursing homes, or assisted living facilities.
  • Strong clinical skills including wound care (sutures), IV infusion therapy, medication administration (including injections), and infection control practices.
  • Familiarity with EMR/EHR systems like Epic or Cerner; experience with ICD coding (ICD-10/ICD-9) is a plus.
  • Knowledge of diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities or dementia care needs.
  • Critical care experience such as ICU or emergency room background is highly desirable; experience in trauma centers or post-acute care settings is beneficial.
  • Ability to perform comprehensive physical examinations and diagnostic evaluations; proficiency in laboratory specimen collection and processing preferred.
