Admissions RN Responsibilities:
  • Complete comprehensive Start of Care (SOC) and admission visits for home health patients.
  • Perform thorough patient assessments and accurately complete OASIS documentation.
  • Develop individualized plans of care in collaboration with physicians and the interdisciplinary team.
  • Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education.
  • Monitor patient progress and communicate changes in condition to the care team.
  • Educate patients and caregivers to promote independence, safety, and successful outcomes at home.
  • Perform approximately 1–3 Start of Care visits per day within a designated territory, allowing for quality one-on-one care.
Qualifications:
  • Current New Jersey Registered Nurse (RN) license in good standing.
  • Minimum of one year of nursing experience; home health experience preferred.
  • Experience with OASIS documentation and completing Start of Care visits required.
  • Comfortable utilizing an EMR; HCHB experience is a plus.
  • Excellent assessment, communication, critical thinking, and organizational skills.
  • Reliable transportation and willingness to travel throughout Ocean County.

Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$55.00 - $62.00 per point
