Providence Care is hiring full time Registered Nurses to join our growing team by providing Case Management services to patients in their homes, assisted living and long term care facilities who are enrolled in our hospice program.

Do you have a passion for helping people? Would you like to help patients and their families create their last best memories? At Providence Care, we provide end of life care to patients and provide support to family members.

Responsibilities:

  • Looking for someone with a Hospice heart and passion for providing end of life care
  • Provide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDG
  • Responsible for overseeing patient plan of care with the collaborating physician
  • Provide supervisory visits and oversight for LPN and CNAs
  • Will conduct start of care visits and collaborate with Care Navigators
  • Provide education to patients, families, and facility staff
  • Provide oversight to Home Health Aides and LPNs

Qualifications:

  • Must have a RN license unincumbered in the state you are applying
  • Reliable transportation and valid drivers license, clean MVR
  • Experience in Hospice/Home Health is preferred
