As an LCMFT, your responsibilities will include:

• Assessing and addressing the therapeutic needs of patients within your assigned care pod, creating individualized treatment plans that align with each patient's unique circumstances and required interventions.
• Facilitating robust discussions regarding behavioral health care coordination with primary care providers and other relevant stakeholders, ensuring comprehensive treatment plans are in place for your clients.
• Monitoring clinical symptoms and observing any side effects or complications due to treatments, with the goal of mitigating symptoms through evidence-based interventions.
• Assisting with psychotropic medication management prescribed by primary care physicians, emphasizing adherence, monitoring side effects, and evaluating treatment effectiveness.
• Documenting patient visits and tracking follow-up outcomes in a systematic registry, which aids in identifying at-risk clients and re-engaging them as needed.
• Collaborating with patients to establish mutual self-management goals, crafting specific objectives, and applying targeted interventions that drive positive outcomes.
• Adapting care plans as necessary for patients who demonstrate insufficient progress, potentially involving referrals to additional services or adjustments in medications.
• Engaging actively with your care team to establish care management plans that involve input from both patients and their families, promoting a holistic view of care.

Your work will focus on a diverse patient base, including individuals experiencing anxiety, depression, relational issues, and other mental health challenges. The population served at Project Reach is rich in variety, encompassing clients from different socioeconomic backgrounds and cultures. Your ability to connect with various demographics and adapt your therapeutic methods will be crucial in providing effective care.

To succeed in this role, you must have:

• An active Licensed Clinical Marriage and Family Therapist (LCMFT) license in the state of Maryland.
• A master’s or doctoral degree in Marriage and Family Therapy or a closely related field.
• Demonstrated clinical expertise, with a focus on serving diverse populations.
• Excellent verbal and written communication skills, alongside strong organizational capabilities to manage multiple patients effectively.

While the following qualifications are not mandatory, they will enhance your application:

• Prior experience in outpatient mental health settings or private practice.
• Familiarity with telehealth platforms and electronic medical records (EMR) systems.

Project Reach offers a competitive compensation package ranging from $80,000 to $90,000, based on experience and professional qualifications.
