Work Environment

Key Responsibilities
  • Act as a vital liaison between clients, families, and outpatient providers, ensuring continuity and coordination of care.
  • Engage with insurance companies for pre-certification and utilization management, advocating for client needs with a keen understanding of insurance processes.
  • Participate in detailed discharge and aftercare planning, creating tailored care plans that address clients' ongoing needs and support their transition.
  • Provide therapeutic meal support and model healthy relationships with food, utilizing techniques that empower clients to improve their eating habits and emotional connections with food.
  • Deliver safe, empathetic, and affirming care, adapting approaches to fit each client's unique context, ensuring an inclusive environment that addresses all aspects of their identities.
Patient Population / Client Focus

Your work will primarily focus on older adults facing various life challenges, including those requiring hospice care. In this role, you will tackle complex emotional and psychological issues, taking into account the unique narratives and identities of each client. Qualifications
  • Master's degree in Social Work from an accredited institution.
  • A valid LCSW license in the state of Maryland is required.
