Key Responsibilities
  • Conduct thorough psychosocial assessments and develop tailored treatment plans based on individual client needs, regularly modifying plans in response to patient progress and challenges.

  • Work in conjunction with a multidisciplinary care team, including social workers, psychiatrists, and other mental health providers, to devise comprehensive discharge strategies that promote continuity of care.
