• Conduct thorough assessments and evaluations to identify patient needs, strengths, and challenges, ensuring a comprehensive understanding of their mental health journey.
• Facilitate effective communication amongst clinical team members, caregivers, family members, and associated parties to provide ongoing updates on patient progress.
• Develop and present individualized treatment plans that include therapy sessions, home visits, and progress tracking to supervisors, ensuring all interventions are tailored to each patient's specific needs.
• Conduct home visits as necessary to perform in-home assessments, gather data, and consult with families, ensuring that care plans are implemented effectively.
• Actively participate in Interdisciplinary Team Meetings to collaborate on patient care strategies and share insights into treatment methodologies.
• Prepare detailed monthly and quarterly progress reports that reflect treatment effectiveness and client engagement, adhering to all legal and ethical standards.
• Develop and conduct training sessions for team members on best practices in the execution of treatment plans and documentation protocols.
• Ensure that records are meticulously maintained and accessible for verification and tracking of patient outcomes, supporting transparency and accountability in care.
• Serve as an essential liaison between the community and mental health resources, assisting patients in reaching their developmental goals and increasing awareness of mental health services.
• Contribute to the creation of targeted training materials aimed at enhancing the scope of individual programs and services.

You will work with a diverse array of clients, including both adults and youth. Our patient population may range from individuals facing mental health challenges to families seeking therapeutic support for relationship issues, trauma recovery, or substance misuse. Your ability to understand and empathize with varied client experiences is key to facilitating their healing processes.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Currently holds an active and unrestricted Professional Counselor (LPC) license in the state of Oregon.
• A minimum of two years of experience in a counseling context, preferably within a clinical setting, is mandatory.
• Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse is highly preferred.
• Strong awareness of legal and ethical practices in mental health counseling is essential.
• Exceptional interpersonal and communication skills with the ability to build rapport with a wide variety of clientele.
• Demonstrated experience working collaboratively within a healthcare team to achieve optimal patient outcomes.
• Familiarity with Electronic Medical Records (EMR) software and related technologies is an advantage.

• Experience working with diverse populations and understanding cultural competency in therapeutic practice.
• Documented history of implementing evidence-based practices in treatment plans.

• Salary ranges from $80,000 to $100,000 annually, commensurate with experience and caseload.
