Key Responsibilities
  • Conduct thorough assessments and evaluations to identify patient needs, strengths, and challenges, ensuring a comprehensive understanding of their mental health journey.
  • Facilitate effective communication amongst clinical team members, caregivers, family members, and associated parties to provide ongoing updates on patient progress.
  • Develop and present individualized treatment plans that include therapy sessions, home visits, and progress tracking to supervisors, ensuring all interventions are tailored to each patient's specific needs.
  • Conduct home visits as necessary to perform in-home assessments, gather data, and consult with families, ensuring that care plans are implemented effectively.
  • Actively participate in Interdisciplinary Team Meetings to collaborate on patient care strategies and share insights into treatment methodologies.
  • Prepare detailed monthly and quarterly progress reports that reflect treatment effectiveness and client engagement, adhering to all legal and ethical standards.
  • Develop and conduct training sessions for team members on best practices in the execution of treatment plans and documentation protocols.
  • Ensure that records are meticulously maintained and accessible for verification and tracking of patient outcomes, supporting transparency and accountability in care.
  • Serve as an essential liaison between the community and mental health resources, assisting patients in reaching their developmental goals and increasing awareness of mental health services.

Qualifications
