Role Overview: Nurse Practitioner, Neurology and Memory Care
We are seeking a skilled and compassionate Nurse Practitioner who is licensed in Massachusetts* to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.
*You must live in and be licensed in Massachusetts and you will work remotely from MA.

Job Description
โ€ข Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
โ€ข Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
โ€ข Partner with our neurologist(s) and broader care team to ensure continuity of care
โ€ข Educate patients and families on diagnoses, treatment plans, and disease progression
โ€ข Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
Key Accountabilities
โ€ข Active, unrestricted NP license in Massachusetts; certification in geriatrics, adult-gerontology, or family practice preferred
โ€ข 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
โ€ข Strong clinical judgement and clinical documentation skills
โ€ข Comfortable managing a full patient panel independently with physician collaboration
โ€ข Excellent communication skills with patients facing cognitive decline and their caregivers
