• Conduct comprehensive assessments of patients' psychological and emotional needs, utilizing standardized tools and methodologies to inform your approach.
• Develop, implement, and manage personalized treatment plans that focus on evidence-based therapeutic interventions.
• Utilize therapeutic modalities such as Cognitive Behavioral Therapy (CBT), trauma-informed care, and mindfulness-based strategies to facilitate positive patient outcomes.
• Maintain meticulous, accurate, and confidential records in accordance with HIPAA regulations, ensuring patient privacy and data security.
• Collaborate closely with our specialized treatment team, including nurses, physicians, and other social workers, to coordinate holistic care and treatment strategies.
• Stay abreast of the latest research and best practices in the field of social work, engaging in continuous professional development and training opportunities.
• Participate in regular team meetings and case conferences to share insights and enhance collaborative treatment efforts.

In this role, you will primarily work with a diverse population of patients at the hospice center. These individuals often experience significant psychological distress related to end-of-life issues, chronic illness, and bereavement. Your expertise will provide essential emotional support, helping clients and their families navigate these challenging periods with dignity and grace.

• Master's degree in Social Work from an accredited institution.
• Current and active LCSW license in the state of Connecticut.
• Strong clinical knowledge and understanding of social work principles, ethics, and practices.
• Demonstrated proficiency in conducting assessments and developing treatment plans tailored to client needs.
• Excellent verbal and written communication skills, allowing for effective collaboration with team members and clear documentation of patient interactions.
• Ability to work independently while managing a full caseload of clients.

• Experience working with terminally ill patients or in a hospice setting is highly desirable.
• Familiarity with telehealth platforms and remote service delivery.
• Background in managing trauma, depression, and anxiety-related disorders within a clinical environment.
