The Behavioral Health Clinical Navigator Registered Nurse is responsible for reviewing, processing, and ensuring the accuracy of clinical documentation. The Clinical Navigator is responsible for suggesting and implementing changes, educating field staff on documentation requirements, and ensuring state and federal requirements are consistently followed. The Behavioral Health Clinical Navigator RN optimizes quality of care across the clinical departments by undertaking efforts to review clinical competency and ensure regulatory adherence.

What is Required?

  • High School Diploma or GED
  • Associate degree in a Nursing related field required
  • 2 years of experience as a nurse in an appropriate clinical care setting
  • Experience in management, Infection control, and Home Health and Hospice surveys is preferred
  • Experience in a home health or hospice setting is preferred
  • Home Care Home Base knowledge and platform familiarity with PointCare
  • Working knowledge of Federal and State rules and regulations for Medicare and Medicaid
  • Valid driver's license and insurance
  • Reliable transportation to perform job tasks
