The Licensed Clinical Social Worker position is a remote role, allowing for flexibility while still maintaining a connection with a cohesive and collaborative team dedicated to mental health care. • Conduct thorough psychosocial assessments to evaluate each patient's mental health status, strengths, and specific needs, ensuring a comprehensive understanding of their unique situations.
• Provide therapeutic services utilizing evidence-based modalities, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to individual patient requirements.
• Engage in multidisciplinary collaboration by working closely with psychiatrists, nurses, and other healthcare professionals to ensure coordinated and cohesive care plans.
• Assist patients and their families in comprehending diagnoses, treatment options, and medication, fostering a supportive and informative environment.
• Deliver individual, group, and family therapy sessions utilizing proven therapeutic approaches to address various mental health concerns.

The Licensed Clinical Social Worker role will cater primarily to individuals facing severe or terminal health conditions within our hospice center. You will engage with patients grappling with end-of-life challenges, mental health disorders, and a variety of emotional struggles, providing empathetic and specialized care to help them navigate their circumstances. Your contributions will directly impact not just the patients but also their families, helping them understand and cope during difficult transitions.

• A Master's degree in Social Work from an accredited program, demonstrating a strong educational foundation in clinical practice.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory to practice within Connecticut.
• A solid understanding of mental health disorders, treatment modalities, and crisis intervention strategies to effectively support patients.
