Work Environment

This position is primarily remote, allowing our Licensed Clinical Social Worker the flexibility to provide services from their own workspace while remaining fully integrated with our hospice care team. Key Responsibilities
  • Conduct thorough psychosocial assessments that consider both clinical records and personal interviews to understand the emotional and social factors affecting the patientโ€™s end-of-life care.
  • Provide individual and family counseling, assisting them in coping with grief, fear, and adjustment to terminal diagnoses as they navigate complex emotional landscapes.

Qualifications
