Key Responsibilities
  • Assist in the planning and organization of outpatient programs tailored to individual and community needs.
  • Communicate effectively with physicians and other healthcare team members to monitor and report on patient progress, discussing concerns, and identifying potential problems.
  • Maintain comprehensive and accurate clinical records, including timely documentation of progress notes in accordance with clinic policies.
  • Collaborate with colleagues to develop and implement evidence-based treatment plans, including tailored discharge plans following thorough psychosocial assessments.
  • Provide counseling and crisis intervention services, assessing and treating emotional and behavioral issues that patients may encounter.
  • Engage in consultations with patients' families and support systems to facilitate seamless treatment and improve continuity of care as patients transition out of our services.
Patient Population / Client Focus

Our clinic primarily serves high-risk patients who may face complex mental health issues. This includes individuals grappling with trauma, anxiety, depression, and various behavioral disorders. Your expertise will help these individuals and their families overcome significant personal and environmental challenges, thereby optimizing their interactions with medical care and enhancing their overall health outcomes.

Qualifications
  • A minimum of 2 years of experience in an outpatient mental health setting, demonstrating effective counseling and client interaction skills.
  • A Master's or Doctorate degree in Social Work, Psychology, or a related field is required, providing the foundational knowledge necessary for this role.
  • Active state licensure as a Licensed Professional Counselor (LPC) is mandatory.
  • A strong ability to work independently while also being part of a larger healthcare team dynamics.
  • Proficiency in written and spoken English is essential for effective communication with patients and colleagues.
  • Competency in technology, particularly with MS Office and electronic health record systems, is important for maintaining accurate patient documentation.
Preferred Experience
