• Conduct comprehensive clinical assessments for patients and develop individualized treatment plans focusing on emotional, social, and practical support.

Your primary focus will be on patients with terminal illnesses and their families, fostering an understanding of the emotional complexities surrounding end-of-life issues. • Master's degree in Social Work from an accredited institution, ensuring a solid foundation in social work theory and practice.
• A valid LCSW license in the state of Vermont, demonstrating professionalism and adherence to legal standards.
• Strong knowledge of solution-based psychotherapy techniques that are particularly effective in a hospice setting.
• Exceptional communication skills, both verbal and written, to effectively interact with patients, families, and interdisciplinary team members.

• A minimum of 2 years of professional experience in a social work role, particularly in healthcare settings, with exposure to hospice or palliative care preferred.
• Demonstrated ability to navigate sensitive conversations about death and dying compassionately.
• Familiarity with electronic health records (EHR) systems to manage clinical documentation efficiently.
