Job Responsibilities
  • Provide skilled nursing care to patients in their homes according to the individualized plan of care.
  • Perform comprehensive patient assessments and monitor changes in patient condition.
  • Administer medications and treatments as ordered by the physician.
  • Provide wound care, pain and symptom management, and other skilled nursing services.
  • Educate patients and families regarding disease management, medications, safety, and plan of care.
  • Coordinate care with physicians, therapists, aides, social workers, and other members of the interdisciplinary team.
  • Complete accurate and timely clinical documentation in accordance with agency, Medicare, and regulatory requirements.
  • Participate in care coordination, case conferences, and other required meetings.
  • Provide compassionate, patient-centered care while maintaining professional standards.
Qualifications
  • Current and unrestricted Texas Registered Nurse (RN) license.
  • Graduate of an accredited nursing program.
  • Current CPR/BLS certification.
  • Previous Home Health or Hospice experience preferred but not required.
  • Strong clinical assessment, communication, and organizational skills.
  • Ability to work independently and manage patient visits efficiently.
  • Reliable transportation and the ability to travel within the assigned service area.
  • Commitment to providing compassionate and professional patient care.
