Position: Registered Nurse (RN) - Home Health
Employment Status: Full-Time, Part-Time, or Per Diem
Location: Caseload tailored to your preferred location.
( OPO Health is licensed in over 15 counties. )
Essential Job Functions and Responsibilities:
Key Responsibilities
Patient Care:
  • Conduct comprehensive assessments of patients and families to identify home care needs, including physical assessments and health history.
  • Reevaluate patient nursing needs regularly.
  • Develop, implement, and update individualized care plans based on nursing diagnoses, patient goals, and evolving conditions.
  • Administer prescribed medications and treatments.
  • Provide education to patients and families regarding care and health management.
  • Identify and implement discharge plans as part of the care process.
  • Serve as Case Manager when designated by the Clinical or Nursing Supervisor, coordinating care for assigned patients.

Communication:
  • Maintain accurate clinical notes and update the primary physician as needed.
  • Collaborate with physicians to report changes in patient conditions and obtain necessary orders.

Additional Duties:
  • Arrange for equipment and services essential to patient care.
  • Supervise, train, and evaluate home health aides.

Requirements
Qualifications and Skills:
  • Active and unencumbered California RN license.
  • Valid California driver's license and proof of auto insurance.
  • At least one (1+) year of relevant nursing experience.
  • Current CPR certification.
