Anthony L. Jordan Health Corporation is seeking a Board Eligible or Board Certified Family Medicine Physician to join one of the nation's oldest Federally Qualified Health Centers serving the Rochester community for more than 120 years.

Requirements
Responsibilities
  • Provide comprehensive outpatient primary care to pediatric, adult, and geriatric patients.
  • Perform preventive care, acute care visits, chronic disease management, and health screenings.
  • Develop and maintain continuity of care for an assigned patient panel.
  • Collaborate with nurses, behavioral health providers, pharmacists, care managers, and other interdisciplinary team members.
  • Refer patients to specialty care and community resources as appropriate.
  • Utilize evidence-based medicine and participate in quality improvement initiatives.
  • Serve as a preceptor for residents and students as assigned.
  • Maintain accurate and timely documentation in the electronic health record.
  • Participate in departmental meetings and organizational initiatives.

Education and Experience Required:
• Possession of a license to practice as a medical doctor in New York State.
Licenses and Certifications:
• Board eligibility or Board certification.
• Must obtain credit of CME as required.
• Must meet privileging and credentialing requirements for Physicians.
• Infection Control certificate.
• BLS through the American Heart Association.
Special Skills, Knowledge Required:
• Ability to administer impartial treatment to a wide cross segment of the socio-economic spectrum.
• Ability to work under conditions of stress.
• Skill in interpersonal relationships.
• Proficiency in MS Office Applications (Word, Access, Excel, Power Point, Outlook) and eClinicalWorks or other electronic health record.
