Cherish Hospice is seeking an on the road CNA Hospice Aid in the Kosciusko and surrounding areas. As a hospice aide you can help enhance the quality of life for the patient by providing personal care and related assigned services to hospice patients directly.

CNA Hospice Aide Responsibilities:

  • Provide personal care according to the patient’s needs. Carry out the duties assigned to increase the patient’s level of comfort, and to maintain personal hygiene and a safe environment for the patient.

  • Care provided is in accordance with an established plan of care and under the supervision of a RN

  • Observe the patient, report observations and document observations and care performed.

  • Document the care provided and the patient’s response to care in accordance with policies and procedures and regulatory standards.

  • Provide timely documentation of patient services based on plan of care.

  • Adhere to Company policies and procedures and regulatory compliance requirements and directives.

CNA Hospice Aide Requirements include but are not limited to:

  • Travel as necessary to service patients in their residence of care, which includes home, assisted living facilities, long term care facility, and hospital.

  • CNA License (Required)

  • Driver's License (Required)

  • Hospice experience (Preferred)

  • Geriatrics experience (Preferred)

  • Hospice and Palliative Medicine (Preferred)
