RN MDS Coordinator – Skilled Nursing
$36.50–$49.00/hour | Full-Time

What You'll Do:

  • Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments
  • Ensure compliance with Medicare, Medicaid, OBRA, and other payer program requirements
  • Ensure MDS assessments accurately reflect each resident's physical, mental, and psychosocial status
  • Verify documentation supports services provided and reimbursement accuracy
  • Support interdisciplinary collaboration for accurate care planning, assessment completion, and ongoing case management coordination
  • Collaborate with clinical and administrative teams regarding admissions, transitions of care, and payer communication related to skilled nursing services
  • Monitor assessment schedules and ensure regulatory timelines are met
  • Assist with quality improvement initiatives and survey readiness efforts

Required Qualifications:

  • Graduate of an accredited school of nursing with a current RN license in the state of employment
  • At least 2 years of MDS 3.0 and case management experience in a skilled nursing or post-acute care setting required
  • Ability to work independently while collaborating effectively with interdisciplinary teams
  • Strong organizational, communication, and critical thinking skills
  • Working knowledge of Microsoft Office applications
  • Preferred Qualifications
  • Previous long-term care or skilled nursing experience
  • RAC-CT certification or willingness to obtain within six months of hire and maintain certification requirements
