Job description

A Typical Day for a Registered Nurse Case Manager with WesleyLife At Home Hospice:
As an RN Case Manager with WesleyLife at Home Hospice, each day begins with purpose-the opportunity to bring comfort, dignity, and compassionate support to patients and families navigating one of life's most meaningful journeys. During your visits as a Registered Nurse Case Manager you will:
  • Travel to each patient to conduct thorough nursing assessments to monitor symptoms, comfort levels, and overall well-being
  • Manage pain and other symptoms using evidence-based hospice practices
  • Coordinate medications, treatments, and supplies to support patient needs
  • Prepare and update individualized care plans in collaboration with the interdisciplinary team
  • Provide education on what to expect throughout the hospice journey
  • Teach family caregivers how to provide safe, comfort-focused care
  • Offer a listening ear and calm presence during moments of uncertainty

Open Shifts Available:
  • Full-Time Monday-Friday, Back-up on-call Weekday and weekends

Qualifications:
  • Valid, unrestricted Iowa driver's license.
  • Current vehicle insurance.
  • Current license as a registered nurse in the state of Iowa.
  • Two years of full-time nursing experience required, with one year of hospice experience preferred.
