• Conduct thorough assessments of clients’ social, emotional, and environmental challenges to generate data-driven insights.
• Develop customized service plans featuring interventions designed to address identified goals, with ongoing monitoring of progress.
• Provide counseling to individuals and families navigating complex life challenges, including but not limited to grief, adjustment difficulties, and mental health issues.
• Work collaboratively with clients to establish personal goals and connect them with available community resources that can enhance their quality of life.
• Engage in crisis intervention, conduct risk assessments, and formulate safety plans when necessary to ensure client safety and well-being.
• Build and maintain collaborative relationships with community organizations to advocate for additional resources and comprehensive support for clients.
• Participate actively in community outreach and promotional activities to raise awareness of available services and foster engagement.
• Involve your expertise in case conferences, team meetings, and ongoing professional training to enhance both team and individual performance.

As a Licensed Clinical Social Worker at Legacy Healing Center, you will engage with a diverse range of individuals and families facing critical life transitions, particularly those involved with hospice care. The populations served may include individuals diagnosed with terminal illnesses, families in active grief, and those requiring support with mental health challenges intertwined with chronic health issues. Your role is pivotal in navigating the social determinants of health that often complicate these situations.

• A Master’s degree in Social Work from an accredited institution is mandatory.
• Possession of a valid and active state license as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of social and economic justice issues and trauma-informed care principles.
• Exceptional communication skills, cultural competence, and effective problem-solving abilities are essential in serving our diverse client base.

• Previous experience in a social service agency, hospice, or similar setting is highly preferred.
• Familiarity with community resources, social work methodologies such as motivational interviewing, and case management techniques will be advantageous.
