Job description

We are seeking qualified candidates for the Full-Time Psychiatric Rehabilitation Specialist position. The position is responsible for providing Psychiatric Rehabilitation Services (PRS) at facility-based and/or community-based locations as assigned. Facilitate groups/units to enhance an individual’s knowledge to develop, enhance, and retain skills and competencies in living, learning, working, and socializing. Help individuals receive the level of and/or have access to the services and supports they choose, so that they can live in the environment of their choice. Monitor and track progress of individuals in assigned caseload; participate in the development of individualized rehabilitation plans.

Other Responsibilities, but not all-inclusive:

•Coordinate behavioral health care services to individuals; maintain and monitor an assigned caseload of individuals.
• Provide and facilitate an individual’s skill development and support, which includes daily living, socialization, educational and vocational supports and opportunities, and leisure and recreation activities planning, including community integration and connections. Provide counseling if applicable.
• Maintain the clinical chart for the individual to include: assurance that written assessments are completed, preparation of written service or treatment plans, ensuring documentation of services provided, and responding to treatment.
•Conduct and participate in the Individualized Rehabilitation Plan (IRP), treatment planning, recovery, community integration, and case conferences with staff/natural supports, and other agencies.
•Complete, maintain, and submit required documentation. Develop, review, and revise forms, procedures, and standards of care in accordance with accepted professional practice.

Qualifications:

•The ideal candidate will obtain CPRP certification within 1 year of hire; have a Bachelor’s degree and 2 years of work experience in mental health direct service, 1 year of which must be in Psychiatric Rehabilitation Services (PSR). A master’s degree is required if the position provides therapy.
•Certified Psychiatric Rehabilitation Practitioner (CPRP) certification required; must be obtained within two years of hire if not currently possessed.
•Must have excellent oral and written communication skills, as well as be detail-oriented and able to complete a high volume of documentation in a fast-paced environment.
•Strong computer skills are required, as well as Internet research abilities.
•Ability to work independently and as part of a team.
•Knowledge of the mental health system and supports.
•Valid driver’s license with an acceptable driving record required.

Scope:
•Frequent talking and listening.
•Requires lifting and/or carrying and the ability to fold, kneel, stack, stoop, and bend, which may include direct client intervention.
•Must be available for local travel.
•May require crisis intervention for emergencies.
•Participation in on-call coverage as applicable.
•May be required to report to work during inclement weather.
