RN Case Manager Hospice

Job Summary:

Shalom Hospice of Washington is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Case Manager in and around Kitsap County. As a RN Case Manager, you will plan, organize, and direct hospice care for terminally ill patients and their families, providing physical, emotional, and spiritual comfort and support. Essential Job Functions/Responsibilities:

  • Patient Care:

    • Completes comprehensive assessments of patient and family to determine hospice needs

    • Provides professional nursing care utilizing the nursing process

    • Initiates and revises plan of care as patient status and needs change

  • Communication:

    • Maintains accurate and relevant clinical notes regarding patient's condition and care given

    • Communicates with physicians, community health related persons, and families/caregivers

    • Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency

  • Additional Duties:

    • Participates in on-call duties as defined by the on-call policy

    • Supervises ancillary personnel and delegates responsibilities when required

    • Fulfills the obligation of requested and/or accepted case assignments

Position Qualifications:

  • Graduate of an accredited school of nursing

  • Registered Nurse, with a License in good standing to practice

  • Minimum of one (1) year of experience as a professional nurse within the last three (3) years or a baccalaureate degree in nursing and minimum of two (2) years' experience

  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, and nursing skills per competency checklist

  • Ability to lift, position, and/or transfer patients, and to lift supplies and equipment

  • Possesses and maintains CPR certification
