Job description

Eventide Hospice in Grand Island, NE is seeking a Hospice Registered Nurse to work 8a-5p Monday to Friday.

The Hospice RN is responsible for planning, organizing and directing hospice care for Eventide Hospice Senior’s/resident’s/client’s.

Essential Functions 

  1. Completes an initial, comprehensive and ongoing comprehensive assessment of Senior/client and family to determine hospice needs. Provides a complete physical assessment and history or current and previous illness(es).
  2. Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
  3. Assesses and evaluates Senior’s/client’s status by:
    • Writing and initiating plan of care that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions
    • Regularly re-evaluating Senior/client and family/caregiver needs
    • Participating in revising the plan of care as necessary, including the Senior/client and the family in the planning process
  4. Uses health assessment data to determine nursing diagnosis.
  5. Initiates appropriate preventative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  6. Provides health care instructions to the Senior/client as appropriate per assessment and plan.
  7. Assists the Senior/client with the activities of daily living and comfort care.
  8. Acts as Case Manager when assigned by Patient Care Coordinator and assumes responsibility to coordinate Senior/client care for assigned caseload.
  9. Completes, maintains and submits accurate and relevant clinical notes regarding Senior’s/client’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  10. Communicates with the physician regarding the Senior’s/client’s needs and reports changes in the Senior’s/client’s condition; obtains/receives physicians’ orders as required.

Ideal Candidate

  • AGE REQUIREMENT: At least age 18.
  • EDUCATION:  Successful completion of a Registered Nurse course of study.  Bachelor’s degree in Nursing preferred.     
  • EXPERIENCE: Two years of acute care/long term care nursing experience within the last five years preferred. Experience in community health and/or hospice preferred.
  • CREDENTIALS:  RN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required. CPR required.  License must be kept up to date at the expense of the TEAMember. CPR certification must be kept up to date at the expense of the TEAMember.  Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required.  Must possess auto insurance at or above Nebraska statutory requirements. Must meet Eventide’s vaccination requirements.

Compensation
At Eventide, we value our employees and offer a competitive starting pay range of $36-40.32 per hour for RNs based on qualifications, experience, and location. Shift premiums and differentials are also available, for select positions, paid on employment status and shift worked.
