Countryside Manor – Fremont, OH
Full-Time, Part-Time & PRN | Night Shift
$8,000 Sign-On Bonus for Full-Time LPNs

Position Summary:
Countryside Manor is seeking compassionate and skilled Licensed Practical Nurses (LPNs) for Night Shift to join our clinical team. In this role, you will provide quality nursing care in a skilled nursing and long-term care setting while working alongside a supportive and collaborative team.

Key Responsibilities:

  • Provide direct resident care including medication administration, treatments, and nursing services

  • Monitor resident conditions and promptly report changes to the appropriate clinical team

  • Supervise and support Certified Nursing Assistants (CNAs)

  • Complete accurate and timely clinical documentation

  • Collaborate with nurses, physicians, and the interdisciplinary team to support resident care plans

  • Promote a safe, compassionate, and resident-centered environment

Schedule Options:

  • Night Shift

  • Full-Time, Part-Time, and PRN opportunities available

  • Every other weekend required
