Exalt Health, an acute rehabilitation hospital, provides an intensive rehabilitation program, and admitted patients must be able to tolerate three hours of intense rehabilitation services per day. Focused on caring for patients with complex rehabilitative needs such as stroke, spinal cord injury, brain injury, head trauma, medically debilitation conditions, neurological disorders, cardio-pulmonary amputations, orthopedic injuries, and multiple major traumas.


Position Summary


The Licensed Vocational/Practical Nurse, under the direction of a Registered Nurse, is responsible for providing nursing care, health teaching, and health counseling of assigned patients during a given shift. The LVN/LPN assures the delivery of care by assisting in the implementation of the interdisciplinary and nursing plan of care.


Essential Duties and Responsibilities


  • As an LPN in our inpatient rehabilitation setting, you will play a vital role in supporting the nursing team and ensuring patients receive high-quality care during their recovery journey. Your responsibilities will include:
  • Patient Care: Assisting registered nurses with patient assessments, vital signs monitoring, medication administration (under RN supervision), and basic care needs such as bathing, dressing, and toileting.
  • Rehabilitation Support: Working alongside therapists to assist patients with activities of daily living (ADLs) and other rehabilitation activities as directed by the care plan.
  • Patient Education: Providing basic patient education on self-care practices and promoting a positive and supportive environment.
  • Documentation and Reporting: Accurately documenting patient care activities, observations, and responses to treatment under the direction of the RN.
  • Maintaining a Clean and Safe Environment: Assisting with maintaining a clean and organized patient care area and ensuring patient safety protocols are followed.

Licenses or Certifications

  • Current, unencumbered license to practice vocational nursing in the state where currently practicing.
  • Current BLS via the American Heart Association or American Red Cross required.
  • Current ACLS certification via the American Heart Association or American Red Cross a plus.

  • Graduate of an accredited school of professional vocational nursing.
  • Strong clinical skills in basic patient care, medication administration (under RN supervision), and vital signs monitoring.
