Sterile Compounding Technician, Home Infusion

Position Summary

We are seeking an experienced Sterile Compounding Technician to join our cleanroom team. MUST HAVE COMPOUNDING EXPERIENCE. The technician will prepare compounded sterile preparations, primarily TPN admixtures and IV antibiotics, in accordance with USP <797> standards, ACHC accreditation requirements, and Pharmko policies and procedures. This is a hands-on compounding role for a technician with demonstrated home infusion experience who takes pride in precision, aseptic technique, and clean documentation.

Key Responsibilities
  • Compound patient-specific TPN admixtures using automated compounding devices and gravimetric verification

  • Prepare IV antibiotics and other compounded sterile preparations for home infusion patients, including syringes, elastomeric devices, and IV bags

  • Perform aseptic manipulations within ISO 5 primary engineering controls in accordance with USP <797>

  • Complete and maintain compounding records, lot and expiration documentation, and beyond-use dating in the pharmacy system

  • Perform cleanroom cleaning and disinfection per schedule and document completion

  • Participate in required competency assessments, including gloved fingertip sampling, media fill testing, and aseptic technique evaluations

  • Conduct in-process and final checks of components, calculations, and labeling under pharmacist supervision

  • Manage sterile compounding inventory, including receipt, storage, rotation, and lot tracking of drugs and supplies

  • Support environmental monitoring activities and report any excursions or concerns to the pharmacist-in-charge

  • Maintain garbing, hand hygiene, and personnel conduct standards within the cleanroom suite

Required Qualifications
  • Minimum 1 year of sterile compounding experience in a home infusion pharmacy setting (REQUIRED)

  • Hands-on experience compounding TPN and/or IV antibiotics

  • Working knowledge of USP <797> standards and aseptic technique

  • Ability to commute reliably to Lawrence, NY 11559

  • Strong attention to detail, accurate pharmaceutical calculations, and clean documentation habits

  • Ability to stand for extended periods and work within cleanroom garbing requirements

Preferred Qualifications
  • New York State pharmacy technician registration

  • National certification (CPhT through PTCB or NHA)

  • Experience with automated TPN compounders (such as ExactaMix or similar)

  • Familiarity with home infusion pharmacy software and lot tracking workflows
