Summary

Facilitates patient flow into, through and out of the Cape Fear Valley Health system.Major Job Functions

The following is a summary of the major essential functions of this job. The incumbent may perform other duties, both major and minor, that are not mentioned below. In addition, specific functions may change from time to time:

  • Demonstrates standards of performance (ownership, teamwork, communication, compassion) that supports principles of service excellence

  • Communicates with internal and external stakeholders using problem-solving skills to facilitate patient placement and transfers into and out of Cape Fear Valley Health System

  • Receives physician orders on new admissions/transfers

  • Monitors bed availability and coordinates bed placement, transfers, and discharges

  • Maintains a list of patients awaiting transfer into Cape Fear Valley Health System

  • Serves as a liaison for patients, families, physicians, and staff related to patient transfers, bed availability and patient flow issues

  • Facilitates effective utilization of technology available for expediting patient flow through the health system

  • Collaborates with Nursing Leadership, Care Coordination, and individual Units to facilitate patient throughput

  • Other duties as assigned

Minimum Qualifications

The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:

Education and Formal Training:
  • High school diploma or equivalent required

  • EMS credential preferred

Work Experience:
  • Computer experience required

  • Hospital and/or business office experience with specific focus in patient logistics preferred

  • EPIC experience preferred

Knowledge, Skills, and Abilities Required:
  • Ability to communicate needs and concerns in a constructive and positive manner.

  • Ability to prioritize work and maintain control in stressful situations

  • Exhibits creativity and flexibility in problem solving

  • Ability to collaborate with a variety of individuals and disciplines

  • Ability to communicate and resolve patient concerns

Physical Requirements:
  • Visual and auditory acuity required

  • Able to articulate clearly and succinctly

  • Work requires long periods of standing, sitting, or walking

  • Must have command of the English language

  • Visual and auditory acuity
