What You’ll Do
  • Personalized Home Visits: Perform health evaluations and deliver individualized care in patient homes.

  • Holistic Assessments: Evaluate physical, psycho-social, and home environment factors to tailor care plans.

  • Empower & Educate: Teach patients and family caregivers how to manage health conditions effectively.

  • Team Collaboration: Work alongside multidisciplinary teams to ensure complete, continuous care.

  • Documentation & Leadership: Complete timely visit reports and provide supportive guidance to home care aides.


What We’re Looking For
  • License: Active RN license in good standing (Virginia state).

  • Education: Graduate of an accredited nursing program (ADN or BSN).

  • Certifications: Active CPR / BLS certification.

  • Experience: 1+ years of home health or home care experience preferred.

  • Travel Requirements: Valid driver’s license, reliable vehicle, and active auto insurance.

  • Skills: Strong clinical judgment, exceptional communication, and a patient-first heart.
