In addition, MercyOne North Iowa is a teaching institution. We conduct a Family Medicine Residency, Internal Medicine Residency, Cardiology Fellowship, Hospice and Palliative Medicine Fellowship, PGY1 Pharmacy Residency and a School of Radiologic Technology. MercyOne North Iowa is a clinical training site for approximately 500 students from 127 different colleges enrolled in programs for medical laboratory, nursing, paramedic, rehabilitation, medical assistants, pastoral services, physicians and many other medical fields.

The Care Transition Case Management Coordinator is a professional practice leader responsible for advancing care coordination, patient flow, and discharge planning across the acute care continuum. This role maintains direct case management responsibilities while serving as a clinical and operational lead, providing guidance, mentorship, and subject matter expertise to colleagues performing care transition functions. The Lead partners closely with acute care management leadership and interdisciplinary teams to improve throughput, reduce avoidable days, and support safe, timely, and effective transitions of care. Adheres toMercyOne'sperformance expectations and upholds the Mission, Vision, and Values of MercyOne.

General Requirements:

  • Knows, understands, incorporates, and demonstrates the Trinity Health and MercyOne Mission, Vision, and Values in behaviors, practices, and decisions.
  • Coordinates daily clinical operations and supports coverage through schedule input and issue escalation. Identifies improvement opportunities and recommends solutions.
  • Supports maintenance of clinical protocols by assisting with organization, updates, and communication of changes to staff.
  • Provides informal coaching and mentorship on Care Transitions practices; supplies input to leadership as needed for performance matters.
  • Facilitates timely, cross-functional communication with social services, utilization review, nursing, physicians, and post-acute providers to ensure coordinated, patient-centered care.
  • Participates in hiring, onboarding, and training initiatives, contributes to competency assessment activities, and provides feedback to support leadership decisionmaking.
  • Assists with daily huddles and reinforces key metrics, promoting safety, quality, staff engagement, and customer service
  • Serves as a subject matter expert for complex discharge planning and care coordination cases, providing guidance and consultation to colleagues.
  • Supports staff to enhance discharge planning quality, documentation standards, and overall care coordination effectiveness.
  • Maintains an active caseload, delivering comprehensive case management services across the acute care continuum.
  • Facilitates interdisciplinary collaboration to promote timely discharge, safe transitions of care, and optimal patient outcomes.
  • Assist patients in establishing and maintaining care within MercyOne Clinics and connect patients with appropriate care management, health coaching, and community resource programs.
  • Other duties as needed and assigned by the manager.
  • Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health's Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

Education:

  • Must possess a comprehensive knowledge of nursing, as normally obtained through a Bachelor's degree in Nursing or a related field, and three (3) years of acute care case management or care coordination experience.
  • Current Iowa RN license, (MercyOne Genesis - current Registered Nurse (RN) license in either Iowa or Illinois is required at the time of hire. A secondary RN license for the other state must be obtained within 12 months of the hire date.
  • Must be comfortable operating in a collaborative, shared leadership environment.
  • Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.
